Help arranging care

Choose between private care management, program case management and APD waiver support coordination.

Who this may help: Adults and families who need help understanding options or coordinating several services.

Official information checked Nov 1, 2026. Program staff decide eligibility and access; this guide does not assess care needs.

Decide what you want someone to coordinate

Start with a short list: finding local help, comparing arrangements, keeping appointments organized or coordinating an existing plan. Private care managers and public-program staff have different responsibilities. Find Care Florida provides information and contacts; it does not assess clinical needs or manage a care plan.

  • Who will do the work, and what qualifications and relevant experience do they have?
  • Is the arrangement paid privately or provided through a program?
  • What is included, what is charged separately and who receives information?

Private and program routes

A private care manager may help organize services for a fee. Ask for a written scope, costs, conflicts of interest and an explanation of any referral payment. We have not published a verified private-care-manager inventory; a generic home-care listing does not establish that role.

An Aging and Disability Resource Center can explain local support and program access. If you already have a Medicaid plan or another funded service, ask its designated contact about case management. A directory entry does not establish enrollment, authorization or a choice of case manager.

APD waiver support coordination is a specific role

APD waiver support coordinators help people in that program identify needs, plan services and work within program processes. Use APD’s support-coordinator information and provider search to discuss eligibility, current enrollment and choice. Our dated APD Agency records name the service but do not verify current acceptance.

Do not confuse waiver support coordination with CDC+ consultation. CDC+ has its own self-direction requirements and consultant role.
