Who this may help: People exploring APD CDC+ within the relevant waiver-program pathway.

CDC+ is a specific program
APD Consumer Directed Care Plus is a self-direction route connected to the relevant APD waiver pathway. It is not a universal cash payment or an option automatically available to everyone who needs care. Ask APD or your existing support contact about current entry requirements and the steps that apply to you.
Use the official CDC+ page or its help line, 1-866-761-7043, to check the current process. Older enrollment documents keep their printed dates and should not be treated as a newly verified process.
Understand the work involved
Self-direction can involve choosing and managing workers or services within an approved budget and program rules. Ask about purchasing plans, employer responsibilities, records, payroll arrangements and backup support before deciding whether the route fits.
A representative and a CDC+ consultant have defined roles. Consultation is separate from waiver support coordination, ordinary care management and direct care delivery. A dated consultant record does not establish a place in the program.
- Who can explain eligibility, training and the current enrollment steps?
- What decisions can I make, and what requires approval?
- Who handles records, employment responsibilities and unexpected gaps in support?
Choose support with clear responsibilities
Ask a potential consultant about their current program status, service area and what help they provide. Confirm the arrangement through the program. No program application, financial information or health history is needed to browse this guide.
